Met grote kennis van grondstoffen en gereedschappen maakte hij instrumenten voor andere vaklui, die daardoor betere vaklui werden. Omdat hij een handige en kundige instrumentmaker was, verdiende hij een goed belegde boterham en nog wat lekkers bij de thee. De instrumentmaker had een vriend - wiens beroep even niet ter zake doet, maar waarin hij middelmatig kundig was, waardoor hij weinig klanten had en veel tijd om bij zijn vriend thee met wat lekkers te genieten - die regelmatig voor een praatje bij hem langs kwam. De vriend had een zwaar leven; zijn werk liep niet zo best en zijn gezondheid had rimpels en bobbels en was algemeen krakkemikkig. Vooral zijn gebit gaf hem pijn en ach en wee. Dan weer verloor hij een tand, dan weer had hij een gaatje of een ontstoken wortel. Het lekkers bij de thee was wat hem betreft ook liever zacht. De instrumentmaker, die van nature onderzoekend was, overpeinsde de deplorabele toestand van zijn vriend en zocht naar iets om hem te helpen. Op een dag toen zijn vriend al vroeg op de stoep stond - de instrumentmaker zelf was eigenlijk nog lang niet aan een pauze toe, maar vooruit, voor je vrienden heb je wat over - vertelde hij hem enthousiast dat hij knapperige krakelingen voor bij de thee had; zelf hield namelijk eigenlijk veel meer van lekkers met een stevige beet voor bij de thee. De vriend bedankte hem vriendelijk, maar vroeg of er misschien nog iets van de zachte cake van de vorige dag over was. De instrumentmaker moest hem bekennen dat hij zelf de cake vlak voor het slapengaan nog had verorberd, peinzend over het probleem van zijn vriend. ‘’Maar,’’ zo sprak hij met voorpret in zijn stem, ‘’ik heb iets bedacht waardoor ook jij weer van knapperige krakelingen en ander lekkers met een stevige beet voor bij de thee zult kunnen genieten.’’ Hij pakte een doosje van zijn werktafel en maakte dat, met een verwachtingsvolle uitdrukking op zijn gezicht, voor de ogen van zijn vriend open. De vriend keek bevreemd naar de voorwerpen in het doosje. Voor hem lagen twee halve cirkels met puntige dopjes erop. ‘’Kijk, je haalt die laatste paar tanden ook maar uit je mond, die geven je toch niets dan last en ellende, ik heb daar nog wel een paar aardige tangetjes voor, en dan zet je dit gebit ervoor in de plaats. Deze tanden kunnen nooit ontsteken of gaatjes krijgen en je kunt er de knapperigste krakelingen met gemak mee bijten.’’ De instrumentmaker spoelde de mond van zijn vriend schoon met een fles inmaakrum die hij nog had van zijn experimenten met inmaakgereedschappen, en liet zijn vriend er ook een paar flinke slokken van drinken, zodat er geen infecties vanuit de keel omhoog konden komen. Hij koos een fijn tangetje uit zijn generieke voorraad, trok de overgebleven tanden en stompen en plaatste, voor zijn vriend kon roepen dat het toch wel enigszins pijnlijk was en opperen dat hij wellicht toch nog andere opties had, het zelfgemaakte gebit ervoor in de plaats. Na enig gepruts en wennen at de vriend zijn knapperige krakeling, en waarachtig: het lukte. De knapperige krakeling was erg lekker en de thee zorgde ervoor dat de kruimels tussen zijn gehavende tandvlees en de nieuwe tanden werden weggespoeld. Het nieuws ging als een lopend vuurtje rond in de klantenkring van de instrumentmaker en in die van de vriend en al snel kwamen anderen met probleemtanden naar de instrumentmakerswinkel en vroegen of hij voor hen ook zo’n gebit kon maken. De instrumentmaker voldeed hieraan volgaarne en zijn toch al drukke winkel werd nog drukker. Al snel moest hij filialen gaan openen en leerlingen aannemen om al het werk aan te kunnen. Meer en meer mensen kochten alvast een gebit bij hem vóór zij problemen met hun eigen tanden kregen en na enige tijd begonnen mensen bij voorbaat alvast hun tanden te laten trekken en een gebit van de instrumentmaker te nemen, zodat zij zelfs geen risico zouden lopen op gaatjes en kommer en kwel met hun tanden. In bepaalde kringen was het al snel een vereiste om erbij te horen. De instrumentmaker voer er zeer wel bij. Het aantal filialen nam hand over hand toe en al doende maakte hij aanpassingen en verbeteringen aan zijn basismodel en breidde hij het assortiment uit voor verschillende doelgroepen en omstandigheden. De roem en de zeer vele goed belegde boterhammen en het overdadige lekkers bij de thee, dat hij ermee verdiende, deden hem verlangen naar meer en nog meer en hij begon actief en misschien zelfs licht agressief reclame te maken voor zijn gebit. Nog decennia later, toen het bedrijf allang niet meer door hemzelf werd gedreven, was zijn naam alom bekend en zijn gebitten waren populairder dan ooit. En de vriend? Wel, de vriend was natuurlijk blij met zijn tanden, hoewel die toch ook best nog wel wat werk gaven en hun eigen kommer en kwel als het op schoonmaken aankwam, maar hij miste zijn favoriete gespreksonderwerp en moest naarstig op zoek naar een ander punt in zijn wankele gezondheid om over te klagen. Zijn vriend de instrumentmaker had maar weinig tijd meer voor hem, maar hij vond een houtbewerker die werkelijk geïnteresseerd leek in zijn verhandelingen over de kommer en kwel met zijn knie- en enkelbanden. En zo leefden zij, naar men mag aannemen, nog lang en gelukkig.<br />

Long time ago, there lived an instrument maker . With great knowledge of materials and tools he made instruments for other craftsmen, who became better craftsmen because of it. Because he was a very good instrument maker he earned a good living, cheese on his every slice of bread and a nice snack with his tea as well. The instrument maker had a friend - whose occupation is of no matter know, but which he was not particularly good at, which caused him to have little customers and lots of free time to have a nice cup of tea and snacks with his friend - who every now and then dropped by for a talk. The friend had a rough life; his work didn’t work out so well and his health had bumps and ripples and was shaky all over. His dental area, especially, gave him pain and aches. Loosing teeth, getting wholes and infected roots on and off. If up to him, the snacks with the tea would be soft. The instrument maker, an inquiring man to begin with, thought the deplorable condition of his friend over and over and tried to find something that might help. One day, when his friend presented himself quite early on his doorstep - the instrument maker himself really wasn’t ready for a break yet, but that is what friends are for, after all - enthusiastically informed his friend about the crunchy cracknels he had as snack with the tea; for personally he favoured a snack with a crunchy bite with his tea. The friend said, thanks, but thanks and was any of yesterday’s soft cake left over, by any chance? The instrument maker had to admit to having eaten that last night himself while munching over his friends problems solution. ‘’But,’’ he spoke, anticipatory pleasure colouring his voice, ‘’I’ve thought something up that will make you enjoy crunchy cracknels and other snacks with a crunchy bite with your tea again.’’ He took a little box of off his desk and opened it with happy expectance glittering in his eyes, before his friends eyes. The friend took a puzzled look at the articles in the box. In front of him, two halve circles with little, sharp knobs on it, lay looking at him. ‘’See? Just get those last sickly, hurting teeth, who do nothing but causing you trouble and woe, out of your mouth, I do have some nice little tweezers somewhere around for that, and then just put this dentures in place. These teeth will never decay or hurt and you’ll be able to bite the crunchiest of all cracknels.’’ The instrument maker washed his friends mouth with a bottle of preserving rum which he had standing around since his experiments with preserves makers instruments, and made his friend gulp down a couple of good swigs of it to prevent infections crawling up from his throat. He choose some tweezers from his generic storage, pulled the remaining teeth and stumps and placed, before his friend could yell that it did in fact rather hurt and postulate that he might have other options, the home-made dentures in his mouth. After some fitting and fidgeting the friend ate the crunchy cracknel and, low and behold: it worked just fine. The crunchy cracknel tasted very good and the tea washed out all crumps from between his new dentures and his wounded gums. Soon the news spread around and the instrument makers customers and the few of the friends business came to the instrument maker shop and asked him if he could make them a pair of dentures as well. The instrument maker was most happy to do so and within no time at all he was busier than ever. Within the shortest of time he had to open branches and take apprentices to get all the work done. More and more people to buy an instrument maker pair of dentures before they had dental trouble and shortly after people started to skip the phase of natural dentures at all, have their teeth ripped out and use the instrument maker dentures right away, to avoid the risk of ever developing dental woe and troubles. In certain circles it was just not done anymore to use your natural dentures. The instrument maker went very well. The number of branches grew very quickly and he remodelled when he got reports of annoyances and added specialties for different target groups and people with special needs. The glory and the very big earnings, good for more cheese on his more slices of bread and even more crunchy snacks with his tea, made him yearn for more and more and then some and he started to market his dentures, even a bit aggressively so. After decennia, when he didn’t even run his own business, his name was famous and his dentures more popular than ever. And the friend? Well, the friend, of course, was really happy with his new teeth, even with the extra work and woe and trouble they made for, for the cleaning and all was a bit of a hassle. But he did miss his favourite subject of conversation and had to go looking for another piece of trouble in his shaky health condition. His friend, the instrument maker, had but little time left for him, but he did find a wood worker, who seemed to be quite interested in his monologues concerning his wobbly knee and ankle ligaments. Hij zet zich over zijn eigen intrinsieke impulsen en instincten heen om goed te zijn en goed te doen. Dit is de compassie die ik zo graag zou zien in meer zorgverleners: zorg voor je patiënt/cliënt zonder een eigen agenda te laten prevaleren. The lover that makes you go through fire, makes you sacrifice yourself for the other’s well-being. And about compassion, love for your fellow-humans. Vrije keuze is een groot goed, waaraan niet mag worden getornd. Ouders hebben allemaal hun eigen opvattingen over wat goede opvoeding inhoudt en anders wil lang niet altijd zeggen beter of slechter. Aan de andere kant is er de verklaring van de rechten van het kind, die onder andere stelt dat ouders de verantwoordelijkheid hebben hun kinderen goed te verzorgen en op te voeden, alles te doen wat in hun macht ligt om te zorgen dat hun kinderen goed worden gevoed en zo gezond zijn en blijven als mogelijk is. In die verklaring krijgen overheden de taak om het de ouders mogelijk te maken deze verantwoordelijkheden uit te voeren. Het is dus misschien wel de taak van de overheid ouders ervan te overtuigen dat de keuze van de voeding van hun baby niet vrijblijvend is, want dat deze de gezondheid en in sommige gevallen zelfs het leven van hun kind kan kosten. James Akre stelt in ‘’The problem with breastfeeding’’ dat het kiezen voor borstvoeding net zo min vrijblijvend is als het aan de hand nemen van je peuter als je een drukke straat oversteekt; dat niet te doen is onverantwoordelijk, gevaarlijk en domweg stom gedrag. Aan de andere kant van het spectrum staat de agressieve directe en indirecte reclame van de fabrikanten van kunstmatige alternatieven voor borstvoeding, die de ouders het idee geven dat ze het voor het kiezen hebben en dat de opties min of meer even goed zijn. Dat de keuze voor borst of kunst meer lijkt op de keuze tussen het ene merk of het andere dan op de keuze tussen gezond en ongezond, tussen echt en namaak. Er is dus duidelijk behoefte aan meer informatie, niet aan meer promotie. Om een keuze te kunnen maken, en ja, uiteindelijk is het de keuze van de verantwoordelijke ouders hoe een kind wordt gevoed, verzorgd, gekoesterd en opgevoed, moeten ouders heel goed weten welke opties er zijn om uit te kiezen en wat van elke optie de mogelijkheden en consequenties zijn. Bij die informatie past geen verzachting van factoren om eventuele ongemakkelijke gevoelens bij de ouders te verdoezelen. Maar ook hoort erbij informatie over mogelijke andere alternatieven als de eerste keuze om een of andere reden niet mogelijk lijkt. Want de keuze blijft niet beperkt tot borstvoeding of kunstvoeding, er zijn mengvormen mogelijk, maar er zijn ook andere alternatieven, zoals afgekolfde melk, donormelk of voeden door een andere moeder. Al deze opties hebben mogelijkheden, onmogelijkheden en consequenties en die horen allemaal thuis in goede, eerlijke, feitelijke en wetenschappelijk onderbouwde informatie. En als dat er allemaal is, dan kunnen en mogen en moeten ouders hun keuze maken. Een vrije keuze, maar gebaseerd op volledige kennis van en inzicht in alle opties.</span></div><div style="line-height: 115%; margin: 0cm 0cm 3pt;"><span lang="EN-US" style="font-family: "Verdana","sans-serif"; font-size: 9pt; line-height: 115%;">~*~*~*~*~*~*~*~*~*~*~*~*~*~*~*~*~*~*~*~*~*~*~*~*~*~*~*</span><span style="font-family: "Verdana","sans-serif";"></span></div><div style="line-height: 115%; margin: 0cm 0cm 3pt;"><span lang="EN-US" style="font-family: "Verdana","sans-serif"; font-size: 9pt; line-height: 115%;">At the start of the new year I posed a question on Twitter: ‘’If the choice of parents influences the health of their children, how free a choice do they have?’’ Some people picked up at the discussion. Freedom of choice is a great good, which may not be challenged. Parents all have their own ideas on what is good upbringing and different not automatically means better or worse. On the other hand there is the Declaration of Children’s Rights which, amongst other things, hold parents responsible to take good care of their children, to do everything they are capable of to feed them appropriately and keeping them as healthy and safe as they can. The Declaration holds governments responsible to make sure parents can fulfil their responsibilities. So it may be the Governments responsibility to make parents know that the choice of infant feeding is not as free as they might like to think, but that it can affect the health and in some cases even the life of their children. James Akre points out in his ‘’The problem with breastfeeding’’ that ‘’ … it's roughly equivalent to the role that choice plays in deciding whether to hold a small child's hand as we cross a busy street together, [...] we hold a small child's hand when crossing street, because we know [...] that doing otherwise is redeemably irresponsible, dangerous, culpable and downright stupid.’’ At the other end of the spectrum we find the aggressive, both direct and indirect, marketing by the manufacturers of artificial breastfeeding substitutes, who try hard and often successfully to convince parents that they are the ones to decide and that the options to choose from are more or less equal. They make it seem as if the choice of infant feeding is more like the choice between this brand or that one, then between healthy and unhealthy, real or fake. It is obvious that there is need for more information, not for more promotion. To be able to make a choice, and, yes, eventually it is the choice of the parents how their child will be fed, nurtured and raised, parents need to know exactly which options are available and what the possibilities and consequences of each of those options may be. In this information is no place for softening of factors that might cause feelings of uneasiness in parents. And information needs to be included about alternatives if the first choice seems not to be possible. Because the choices are not restricted to only breast or formula, there are possibilities like combinations or expressed milk, donor milk or wet-/cross nursing. All these options have possibilities, restrictions and consequences all of these have their places in honest, good and scientifically based information. And with all that in place parents can, must and will make their own choices. Free choices, but based upon full knowledge and understanding of all options.</span><span lang="EN-US"></span></div><div class="MsoNormal" style="margin-bottom: 3pt;"><br />

Dit soort vijand-denken kan heel goed de basis zijn voor de huidige toestand van de wereld en de menselijke samenleving. Als we het eens zouden veranderen in: ‘’Nergens is meer vreugde te vinden dan in een door samenwerkende vrienden succesvol uitgevoerde onderneming’’. Samenwerken in plaats van elkaar tegenwerken, elkaar benaderen in de veronderstelling een vriend te ontmoeten en geen vijand, zou wel eens de meest vruchtbare actie ooit kunnen blijken te zijn, zelfs een die de wereld kan veranderen in een betere plaats.<br />

A news report of an infant declared dead, and surviving after being given to mother to hold in skin-to-skin contact, has made major media attention. Prompted by inquiries, I have made this commentary.<br />
<br />
This is an emotive story, but hardly original! Unusual, but occurs ... actually right here in Cape Town just two weeks ago! Susan Ludington-Hoe opens one of her books on Kangaroo Care with a similar anecdote.<br />
<br />
My own research and "hypothesis" on this is based on the fact that to almost all newborn mammals, separation from mother is life-threatening. This activates a very powerful defence response, which is to shut down and immobilise ( freeze and dissociation by vagal nerve activation). Reptiles use this exact same vagal defence mechanism to slow their hearts to levels that would kill mammals, who need more oxygen! As adults, we think that stress increases heart rate because of our sympathetic nervous system, but what is not properly understood is that even full term newborns have very immature sympathetic nervous systems, and premature infants extremely immature. Prems can only dissociate, and if they are stressed before they are born, they may just remain in dissociation ... with dangerously low oxygen levels.<br />
<br />
Our resuscitation technology can force some regulatory oxygen and breathing and blood pressure and temperature ... but it is working against the "autonomic nervous system tide". There is great variability in sensitivity and resilience in all human beings, and some are sensitive and succumb despite our technology.<br />
<br />
What "kangaroo care" does is restore the basic biology for survival. It is "skin-to-skin contact" which is the key, because the deep sensory fibres from the skin go to the "emotional processing unit" of the brain (amygdala), and tells the brain "you are safe". This de-activates the dissociation (un-safe mode), and restores the regulation (safe mode) - which is the real function of the vagal nerve.<br />
<br />
But there may be a paradox in this very case. Circumstances led to this infant being allowed to stay in skin-to-skin contact for a long time, which may in fact have been its saving grace !! Perhaps its tolerance of separation may have been non-existant. But the paradox may work even deeper ... perhaps it was so profoundly powerful in its vagal response to dissociate in order to survive, that it could last long enough in the shutdown state to be allowed to come back to mother! He may therefore be highly resilient, which is why he survived !!!! The World Health Organisation calls this Kangaroo Mother Care, and Mother was the key to this baby's survival.<br />

En dat onderzoek was uitgevoerd volgens de gouden standaarden van wetenschap: de werkzame stoffen in deze kruiden en specerijen werden geëxtraheerd (want je moet wel heel precies afbakenen wat je onderzoekt) en vervolgens gevoerd aan kleine dieren zoals muizen en ratten. En wat blijkt? Als je die diertjes nu maar lang genoeg van die stoffen voert worden ze ziek. Wie had dat nu gedacht? Lang leve de wetenschap.<br />

We can't eat fennel anymore, nor cloves nor cinnamon. These studies were performed following the scientific golden standards: the working compounds of the herbs were extracted (for you must define very carefully what exactly you study) and fed to little animals like mice and rats. Met vier man sterk kwamen de dappere dienders deze crimineel aanpakken. Veel hielp het niet. In onze tijd en cultuur lukt het al een stuk beter met zalfjes en operaties, maar vooral door kleding en gedrag. Nou, wannabe jong toch in elk geval. Vrouwen blijven meisjes met vriendinnetjes en mannen blijven jongens met onbehaarde torso’s. Waar in voorgaande eeuwen kinderen miniatuur uitvoeringen van volwassenen waren, zo zijn vrouwen boven de 20 tegenwoordig uit de kluiten gewassen schoolmeisjes. Inherent aan het blijven steken bij jong en puberaal is dat alles vooral leuk moet zijn en je persoonlijke behoeften moet bevredigen. Nou zou dat allemaal nog niet zo erg zijn, ware het niet dat dat voor veel mensen ook blijft gelden als zij kinderen krijgen die moeten worden verzorgd en opgevoed. Ook dat moet vooral voor de ouders leuk zijn, niet te veel persoonlijke inzet kosten en veel persoonlijke bevrediging geven. Financieel mag het wel wat kosten, want dan heb je ook wat. Opvoeden wordt dan dat wat je doet om je kind een ‘’fantastische jeugd’’ te geven, waarbij het kind vooral leert voor zichzelf op te komen en je moeder je beste vriendin is. Volgens sommige onderzoekers heeft dat geleid tot een ‘’grenzeloze generatie’’ (Lampert en Spangenberg, uitg. Nieuw Amsterdam 2009). Eén uiting van de grenzeloosheid heeft de vorm van een 14 jarig meisje dat de drammerig de hele wereld op zijn kop zet omdat zij kost wat kost in haar eentje op een zeilboot de wereld rond wil. Kan ze dat? Zeiltechnisch vast wel. Heeft ze het nodig? Nee, niet echt. Wat zij nodig heeft zijn duidelijke grenzen waarbinnen zij zich sociaal, emotioneel en intellectueel kan voorbereiden op volwassen worden. Echt volwassen, met een goed ontwikkeld gevoel voor verantwoordelijkheid. En dat doe je door als puber te dromen van heldendaden en heroïsche tochten, je af te zetten tegen die ‘’achterlijke volwassenen’’ die je altijd alles verbieden wat leuk is en goed je best te doen op school, zodat je een vak kan leren en jezelf onderhouden.<br />En wat heeft dat nu met mij als borstvoeding deskundige te maken? Wel die grenzen beginnen al als het kind nog niet geboren is: zeer duidelijke, onwrikbare, fysieke grenzen. Maar ook na de geboorte heeft een kind al gelijk grenzen nodig, in het begin vooral ook fysiek en vooral in de vorm van het lichaam van zijn moeder. Verder is alles wat het kleine kind ‘’wil’’ ook precies wat hij ‘’nodig heeft’’. Elke keer als hij de borst ‘’wil’’, heeft hij hem ook echt nodig: voor eten, voor drinken, voor liefde en nabijheid. Vijf, tien, twintig keer en morgen meer. Maar naarmate het kind groeit en zich ontwikkelt, gaat het dingen willen die het niet echt nodig heeft of die zelfs schadelijk zijn. En dan worden de grenzen die ouders stellen minder fysiek, dan komt het opvoeden om de hoek kijken. Opvoeden met liefde, echte liefde, geen apenliefde met veel cadeautjes en alles goed vinden. Onvoorwaardelijke liefde, maar geen grenzenloze liefde. Want die grenzen heeft een kind nodig, in de peuterpuberteit en in de echte puberteit. It didn’t work out, really. In our time and place it works a bit better by using creams and operations, but especially bij beahviour and clothing. Well, kinda works anyway. Women stay girls with girly friends and men stay boys with hairless chests. While in ages gone by children where miniature adults, nowadays women above 20 remain oversized schoolgirls. Inherent in getting stuck in adolescence is that everything needs to be fun and satisfying personal needs. That shouldn’t be that big a problem, if it would not persist once children the need to be fed, cared for and brought up come into the picture. Bringing up children needs to be fun for parents, not take too much personal commitment and give lots of personal satisfaction. It may cost some money, but that pays off. Bringing up children is about giving them a ‘marvelous childhood’’, teaching her to stand for her rights and mom is her best friend. According to some researchers this led to borderless generation (Lampert en Spangenberg, publ. Nieuw Amsterdam 2009). One example of this borderlessnes is a 14 year old girl that as frantic about solo sailing around the world. Can she do that? Sure, sail-wise she will have the skills. But does she need it? No way, she doesn’t. What she needs are borders and boundaries within she will be able to develop socially, emotionally and intellectually towards adulthood. To be a real adult, with a fair sense of responsibilities. Adolescent way to reach that is by being frantic about borders and boundaries, and stupid parents who deny you everything fun and by working hard at school to learn a profession and maintain oneself.<br />And what has all this t do with me, the breastfeeding specialist? Well, those boundaries start way before birth, in a very physical way. After birth the child needs borders and boundaries, again in a very physical, bodily way, to be precise, mom’s body. All this little one wants is what he needs. Every time he ‘’wants’’ the breast he needs it. Once, ten, twenty times and more tomorrow. But while developing into toddlerhood and beyond what he wants will not stay what he needs al the time. Then the borders are less physically and start being real upbringing stuff. Child rearing with love, real love, not the monkey love with lots of things and presents. Unrestricted love, but not borderless love. For borders, that’s what children really need and keep needing. Borders to kick at and try to stretch in toddler and adolescent years. Niet veel nieuws, de lezers grotendeels op vakantie - tijd voor wat onzinverhalen. Zoals dat over de Internationale Wereld Borstvoeding Week (WBW). Zowel in de papieren als de virtuele media verhalen over het nut van borstvoeding voor die zielige kindjes in de Derde Wereld. Het thema van de Internationale WBW gaat over zuigelingenvoeding in noodsituaties: borstvoeding als overlevingspakket voor de meest kwetsbaren in de getroffen samenleving. Dat kan de Derde Wereld zijn, maar ook alle ander werelden waar natuurrampen, oorlogen en andere noodsituaties kunnen voorkomen. Zelfs een kilometerslange file op de Route du Soleil is een noodsituatie voor een gezin met jonge kinderen. (nee ik ga niet beginnen aan de discussie waarom je dan uitgerekend met je kinderen die kant op wilt op het moment dat miljoenen anderen dat ook willen) <br />Deze manier van berichtgeving suggereert weer dat borstvoeding eigenlijk alleen voor de zielige kindjes in arme delen van de wereld van belang is. Wij hebben immers de door wetenschappers en dokters gesponsorde vervangingsmiddelen voor moedermelk. Feit is natuurlijk dat ook in onze onmatig welvarende maatschappij kinderen baat hebben bij borstvoeding en dat het zelfs ook hier levensreddend kan zijn. Maar in elk geval altijd de moeite waard.<br />Borstvoeding, de moeite waard is dan ook het thema van de WBW in Nederland, die net als in veel Europese landen niet in de komkommertijd wordt gevierd, maar in week 40. Zou de landelijke pers dan ook nog een gaatje kunnen vinden in de berichtgeving? Not much news to cover, most readers celebrating vacation – time for some non-sense stories. Like the one about the International World Breastfeeding Week(WBW). Both in paper and virtual media the same story about the need for breastfeeding for pitiful children in the Third World. WBW theme is about infant feeding in emergencies: breastfeeding as survival package for the most vulnerable ones in regions struck by disasters of any kind. Sure, that can be The Third World, with all of its wars and natural disasters. But it can also be a miles long traffic jam on the Route du Soleil with a car full of young kids. (no, I won’t go into the discussion of why on earth one would be willing to go exactly there with kids at the exact same time as millions of others)<br />This kind of journalism suggest again that breastfeeding is of need for the woeful little children in needy areas only. Don’t we have the scientists and doctors sponsored replacements foods for our precious ones? Fact is, of course, that even our exorbitantly supplied society children will benefit from breastfeeding and that it can even here be lifesaving to breastfeed a child. Anyway, always worth the trouble.<br />And that will be the Dutch WBW theme, that will be celebrated not in Cucumbertime, but in week 40 as in many other European countries. Wonder if the press will find a little spot for commenting on then. Reden genoeg tot klagen, al is het maar over de vragen, de lengte van het examen en de examen omstandigheden in de gymzaal. De <a href="http://www.laks.nl/index.php?catg=1&navID=1">scholierenvakbond Laks</a> registreert als vanouds de klachten. Veel klachten dit jaar gaan over het examen Nederlands voor VWO, met een onhandig gekozen tekst, moeilijke vragen in vergelijking met voorgaande jaren en mogelijk zelfs spelfouten. Het examen scheikunde komt er bij Laks genadig vanaf, maar daar is nu juist inhoudelijk erg veel mis mee. Examenvragen voor voortgezet onderwijs worden vastgesteld door <a href="http://www.cevo.nl/">CEVO</a>, Centrale Examencommissie Vaststelling Opgaven, maar ze stelt de vragen niet zelf op, dit is uitbesteed aan <a href="http://www.cito.nl/">Cito</a>. De CEVO werkt met een hoge mate van zelfstandigheid onder de eindverantwoordelijkheid van de minister. Voor het opstellen van examenvragen wordt regelmatig gebruikt gemaakt van bestaande teksten van kranten, politiek en industrie. En dat is waar het fout kan gaan. Met de gekozen teksten voor scheikunde ging het dus fout. Ernstig fout. Een kwart van het examen scheikunde VWO werd opgehangen aan een commerciële tekst van een grote fabrikant over kunstmatige voeding voor kwetsbare, te vroeg geboren, zuigelingen. <br />De Nederlandse overheid verleent lippendienst aan een gezonde start voor de burgers van morgen: borstvoeding. Ze ondertekende alle resoluties en appendices van de WHO en Unicef over het bevorderen, beschermen en ondersteunen van borstvoeding en het weren van reclame voor alles wat die bevordering, bescherming en ondersteuning tegenwerkt. In dat kader is er een verbeterde Arbeidswetregeling getroffen die het moeders mogelijk maakt om tijdens werktijd hun kind de borst te even of hun melk te kolven. Dat is mooi, daar kan een moeder wat mee, mits natuurlijk de uitvoering ervan is zeker gesteld. Daarnaast is een halfslachtige poging gedaan om de reclame aan banden te leggen, maar daarbij is de wetgever blijven steken in standaardmelken #1 en mag er vrolijk en agressief reclame gemaakt worden voor dieetvoeding, speciale voeding, voeding voor na de eerste zes maanden en flessen en spenen. De bevordering van borstvoeding is in handen gegeven van het semi-commerciële <a href="http://www.voedingscentrum.nl/home.htm">Voedingscentrum </a>en de ondersteuning in handen van de zorgvelerleners. Extra geld of opleiding hiervoor of gespecialiseerde (lactatiekundige) zorg in het standaard zorgverzekeringspakket is volgens de minister niet nodig, alle klachten van ouders en onderzoeken van TNO dat het aan die ondersteuning van de zorg nog al eens schort ten spijt.<br />En nu dus dat examen. Pure reclame voor kunstmatige zuigelingevoeding voor de meest kwetsbare zuigelingen, gericht op de ouders, wetenschappers en zorgverleners van morgen. Gratis reclame voor die fabrikant, die hard op weg is om 's werelds grootste overtreder van de WHO Code voor het aan banden leggen van dit soort reclame te worden. Gratis reclame gegeven door een onder auspiciën van de minister werkende instelling.<br />Onbenulligheid van de opstellers? Liggen de ministeries zo ver uit elkaar dat zij van elkaar niet (hoeven te) weten wat ze doen en beloven? Of werkt de industriële lobby ook voorbij de wandelgangen van Den Haag en Brussel? <br /><br />~*~*~*~*~*~*~*~*~*~*~*~*~*~*~*~*~*~*~*~*~*~*~*~*~*~*~*~*~*~*~*~*~*~*~*~*~*~*~*~*~*~*~*~*~*~*~*~*~*~*~*~*~*~*~*~*~*~*~*~<br /><br />End of term exams - weather is way to nice to sit insite and study, what a waist to spent these sunny days studying. Reasons enough to start complaining - about anything like the exam questions, the length of the exam, and the conditions in the make-shift examroom: the schoolgym. The <a href="http://www.laks.nl/index.php?navID=7&catg=1&sub=0">Junior and Senior Highschool Students Union, Laks,</a> is the place to dump complaints about exams for the last 20 years and this years harvest is overwhelming. Many focussed on the test for Dutch language and literature, which had a somewhat unluckily chosen text, questions of a more difficult level then previous years and probably some spelling mistakes. Chemistry seemed fine according to complaints received at Laks, but that went way out of scope on subject of 1 of 4 parts. Exam questions for High School Exams are defined by <a href="http://www.cevo.nl/">CEVO</a>, Centrale Examencommissie Vaststelling Opgaven, a semi independant organisation under the responsability of the Ministry of Education. The actual writing of exams is delegated to <a href="http://www.cito.com/">Cito</a>. In preparing the exams often existing texts from newspapers, politican parties and industry are used. And that is an opportunity for mistakes. In the chemistry exam this where it went wrong. Really wrong. One out of four of the multi question questions was based on a commercial text from a multinational producer of infant formulae for the most vulnable, pramture, infants.<br />The Dutch government is paying lipservice to the healthy start of tomorrows citizens: breastfeeding. She signed all WHO and Unicef resolutions ans annexes about the promotion, protection and support of breastfeeding and the defence against marketing for all that will endanger this promotion, protection and support. A law was made to make it possible for working moms to breastfeed or express their milk during paid working hours. That's a good thing for a mom, if, ofcourse, it works out at the workfloor as planned. And even a sloppy attempt was made to reduce marketing, but that one got stuck after banning marketing for standard #1 formulae, which left the way wide open for the promotion of special needs, #2 and #3 formulae, bottles and teats.. The promotion of breastfeeding was handed over to the semi-commercial <a href="http://www.voedingscentrum.nl/Voedingscentrum/English">Nutrition Centre</a> and the support to the health care system. According to the Minister extra money or education, or specialised (lactation consultant) breastfeeding care is not needed, allthought complaints from parents and recent research points out that that is not working.<br />And now, to top it up, the chemistry exam. Plain, clear marketing for formula for the most vulnarable infants, aimed at the parents, scientists and health workers of tomorow. Free marketing for the ,anufacteror, who is heading to beccoming the world's #1 WHO Infant Formula Marketing Code Violator. Free marketing given by an intitution that works under the auspicion of the Ministry.<br />Plain stupidness of the exam writers? Are ministries so far apart that they do not (need to) know what each of them does and promises? Eigenlijk een afgekeken vaardigheid. Vrouwen die in hun kindertijd blootstaan aan beelden van vrouwen die kinderen borstvoeding geven, boren hun instinctieve vermogens makkelijker aan dan vrouwen die nog een kind aan de borst zagen. Voor die laatste groep vrouwen is hulp, steun en informatie van groot belang, zowel als voorbereiding als in de eerste tijd na de geboorte. Meerdere onderzoeken geven aan dat een flink deel van de vrouwen die voortijdig (dat wil zeggen eerder dan zij hadden gepland) stoppen met het geven van borstvoeding dat doen omdat zij onvoldoende steun van de hen begeleidende zorgverleners krijgen. Dit is een belangrijke boodschap voor een overheid die zegt borstvoeding te ondersteunen en promoten en dit alleen doet door het Voedingscentrum een subsidie voor promotie te geven en paar wijdmazige wetten te stellen. Meer kinderen langer borstvoeding lukt pas als de zorg beter kan informeren en begeleiden wanneer de keuze voor borstvoeding is gemaakt. Overheid: zorg dat borstvoeding en de begeleiding daarbij een hoge prioriteit krijgt in de opleiding van alle zorgverleners die werken met zwangeren en jonge gezinnen. ~*~*~*~*~*~*~*~*~*~*~*~*~*~*~*~*~*~*~*~*~*~*~*~*~*~*~*~*~*~*~*~*~*~*~<br />Although breastfeeding is an inborn capability of every woman giving birth, it is skill to be learned. Best learned by living within a social surrounding that includes breastfeeding mothers. Birthing women that did not live in such environment have a hard time starting breastfeeding. They need lots of support, information and help to get breastfeeding started and established. Multiple research studies indicate that lack of professional support from health care providers is a major reason for the premature cessation (that is to say – before they intended to) of breastfeeding. This is an important message to a government who says to support and promote breastfeeding and does so by giving limited grants for promotion to the Nutrition Centre And by putting some wide-mazed laws in effect. More children longer at breast fed will only come true if the health care system can better advise, support and assist when the breastfeeding choice is made. Hoe verdraait moet je geest werken wanneer alles waaraan je kan denken bij het zien van een kind aan de borst seks is, en wel een misselijkmakende vorm van seks.<br />En wat schets dan de verbazing dat hetzelfde facebook kennelijk kindermishandeling niet afstotelijk vindt. De internationale titel is ''Mother Support: going for the gold". Dit verwijst natuurlijk naar de ook in 2008 plaatsvindende Olympische Spelen. Een boeiende vergelijking. Olympische Spelen en moeder tot moeder support gaan allebei over prestaties die je doet omdat je het graag doet en belangrijk vindt. Ook het internationale aspect vind je bij beide terug. Sporters en moeders wereldwijd komen samen en werken samen. Zelf het competitie element is voor beide herkenbaar. Wie is de beste, wie scoort het hoogst. Want hoewel het bij beide om het spel gaat, zijn de knikkers toch ook heel aardig om te winnen.<br />Moeder tot moeder support is een Olympiade waard en waardig. Belangeloze inzet van moeders voor moeders en hun kinderen voor een goede en gezonde start in het leven. The international title is "Mother tot mother support: going for the gold". This is a link to the Olympic Games that will take place in 2008, too. A remarkable comparison. Olympics and mother-to-mother support both are about prestations because one wants to so so and think it's important. The international aspect, too, is found in both. Sportsmen and -women and mothers around the world gather and work together. Even the element of competition is a theme in both. Who's the best, who scores highest.
